Information needed from you when you call:

  • Name, address, phone number, child's name, and child's date of birth.

Then What Happens:

We will mail you:

  • an intake form
  • a brochure on FREE preschool program
  • contact Information with preschool options in your area

You send back the intake form:

  • Mail your completed intake form to the following address:

We will process the intake form:

  • When we receive your intake form, we will process it and forward it to the preschool program of your choice.
  • The preschool program will determine eligibility, and will contact you with details on the application & enrollment process.
